Transition Services
- Assist young people with school-to-work transitioning, working with the schools and vocational programs
- Assist people wanting to move from parent homes, group homes, nursing homes, long-term care facilities, etc. to live in the community
- Providing assistance to individuals with significant disabilities who are at risk of entering institutions, so that the individual may remain in the community
- Provide assistance to persons transitioning from correctional facilities into the community

SPOKES can help you choose a magnifier, mark appliances, learn to cook without sight, learn transportation options, learn your rights as a person with a visual impairment, learn to write and sign your own checks, and apply for free operator assistance and communication equipment through the Oregon TDAP program.
